    Near Miss

    (Guidelines from the NDSU Occupational Safety and Evironmental Health office.)

    For a Near Miss, an incident that did not result in employee injury and/or property damage but had the potential for either, the following procedures apply.

    1. The employee will complete their portion of the Near Miss Report and file it with their immediate supervisor.
    2. The supervisor will complete the 'Near Miss Investigation' portion of the form. As a part of this process, the supervisor assesses the near miss incident and if warranted, takes corrective action to prevent re-occurrence.
    3. The supervisor will give the completed Near Miss Report to the NDSU Libraries' Administrative Assistant. S/he will make a copy for the Library's files and forward the original to the University Police and Safety Office.
