For a Near Miss, an incident that did not result in
employee injury and/or property damage but had the potential for
either, the following procedures apply.
- The employee will complete their portion of the Near Miss Report and file it with their immediate supervisor.
- The supervisor will complete the 'Near Miss Investigation'
portion of the form. As a part of this process, the supervisor assesses
the near miss incident and if warranted, takes corrective action to
prevent re-occurrence.
- The supervisor will give the completed Near Miss Report
to the NDSU Libraries' Administrative Assistant. S/he will make a copy
for the Library's files and forward the original to the University
Police and Safety Office.
