    Process For Reporting Work Place Injuries

    (Guidelines from the NDSU University Police and Safety Office.)

    This policy applies to all NDSU employees including student workers while on work time.

    If you are injured and seek medical care:

    1. Notify your immediate supervisor at the time of injury or have a colleague contact him/her while you are in transport to a medical facility.
    2. The supervisor will notify the University Police and Safety Office, 231-7759. A representative from that office will meet you at the medical facility to assist with the Workforce Safety & Insurance process.
    3. Obtain necessary medical care from the MeritCare? Occupational Health Center located at 3838 12th Ave. N., Fargo, as required by Workforce Safety & Insurance.
    The following exceptions can be made to this policy:
    • In emergency medical situations, utilize 911 services and Emergency Room care at the MeritCare Hospital.
    • If you are working later than regular business hours, the MeritCare Walk-In clinic (at the MeritCare South Campus) is available until 9 p.m. seven days a week, or you may obtain medical treatment at an emergency room.
    • If you are away from Fargo on University business, seek care from the nearest medical provider available.
    • If you have designated an alternate medical provider prior to the injury, seek care from that provider.
    1. 4 Complete the NDSU Incident Report Form, have your supervisor complete the Supervisor Section, and return it to the Administrative Assistant within 24 hours of the incident/treatment.

    If you are injured and do not seek medical care:

    1. Complete the NDSU Incident Report Form, have your supervisor complete the Supervisor Section, and return it to the Administrative Assistant within 24 hours of the incident.
